SCHEDULING OPTIONS

PHYSICAL THERAPY VISIT60 mins. Each visit includes assessment and treatment of injury.
CONTINUITY PLAN/ PERFORMANCE WELLNESS COACHINGMany times after an injury has resolved, clients continue to choose to be seen for regular or less frequent sessions to continue to work towards activity goals. These may include specific training for longevity, osteoporosis, optimizing function, improving performance, strength or mobility progressions, sports-specific training, or to improve injury resilience and develop preventative strategies. We will continually assess for any mobility or strength deficits, test for the most appropriate exercise progressions for your custom and specific training program, so that you can be efficient with working towards your goals.

PHYSICAL THERAPY SERVICES

ECLECTIC APPROACH
We recognize that each person heals completely differently, so we use an array of treatment techniques to customize your program.

Initially, we often utilize Mechanical Diagnosis and Therapy principles of repeated movement assessment and joint Mobilization with Movement to better understand if your injury falls into the category of neurophysiologic/ joint dysfunction which usually are rapid responders versus soft tissue dysfunctions which physiologically take more time and effort to remodel to a capacity that match your activity level. This information is vital to help us understand your injury and what it will take to fully heal and meet your goals.

Other techniques we commonly use include Muscle Energy Techniques, Joint Mobilizations, Strengthening and Stretching Progressions, Soft Tissue Massage, Strain-Counterstrain, NAIOMT, PNF, Mulligan and McKenzie techniques.

PHYSICAL THERAPY VISIT DETAILS

All sessions will be an individualized and customized 1-on-1 experience with a Doctor of Physical Therapy.

FIRST VISIT: YOUR EVALUATION
This is usually the longest session at 75 mins, which will allow us to thoroughly assess you. Oftentimes, this will include specific assessment of mobility, strength, neurodynamics and functional movements, so we can identify pain producers through specific tissue, joint and nerve testing. This allows us to understand limitations so we know where to focus treatment. Specific treatment will follow the assessment. At the end of this session, you will be given a specific diagnosis, all questions will be answered, and you will be given a tailored home program, so you can continue to work towards long-lasting improvement and progress towards achieving your long-term goals.

At the end of your first session, the therapist many times recommend the most appropriate follow-up visit package, as this will allow us to progress and layer on next levels, so that we make sure that the injury fully resolves, long lasting changes occur and you becomes resilient to re-injury.

SUBSEQUENT VISITS
These are 60 mins sessions and usually begin with a brief assessment at the start to determine if we are heading in the correct direction, if progression is needed or if we need to change course. This session will be mostly treatment techniques and testing to accurately identify the correct progression level for your home program.

INTAKE FORMS
After your first visit is scheduled, you will be emailed access to an online web portal, where you will be able to log in and fill out an intake form prior to your first session. This will include a thorough medical history form and standard policies and consent to treat information. You are encouraged to thoroughly fill out the medical history including all injuries you have or other pertinent medical history, so that we can better understand you as a whole, as we want to be as holistic in our approach as possible. This allows us to not only work towards getting you pain-free, but also optimize your functional capacity.

DRESS
Please dress in active clothing that allows your body to move. This allows us to be more accurate with your assessment and provide better treatment.

PAYMENT OPTIONS
PhysioMender is an Out-of-Network, fee for service provider. Payment is due at the time of service. Cash, check, credit card, HSA, FSA are accepted forms of payment. We do not directly accept insurance. You can check with you insurance company to see if you have Out-of-Network benefits, and if so, you may request a receipt/ Super Bill (after all visits for the given plan of care are completed) to submit for reimbursement to your insurance company. You CANNOT submit Out-of-Network claims to Medicare, Medicare Advantage Plans, or Medicaid.
