CONTINUITY PLAN | Custom Performance Coaching and Wellness Programs are designed to keep you progressing towards your goals or event, gaining strength, and becoming resilient to injuries (preventative). This is offered after your injury is resolved and regular physical therapy visits end. These can be either a digital monthly plan or more infrequent in person visits with a focus on updating your custom program. |

PHYSICAL THERAPY SERVICES
ECLECTIC APPROACH
We recognize that each person heals completely differently, so we use an array of treatment techniques to customize your program.
Initially, we often utilize Mechanical Diagnosis and Therapy principles of repeated movement assessment and joint Mobilization with Movement to better understand if your injury falls into the category of neurophysiologic/ joint dysfunction which usually are rapid responders versus soft tissue dysfunctions which physiologically take more time and effort to remodel to a capacity that match your activity level. This information is vital to help us understand your injury and what it will take to fully heal and meet your goals.
Other techniques we commonly use include Muscle Energy Techniques, Joint Mobilizations, Strengthening and Stretching Progressions, Soft Tissue Massage, Strain-Counterstrain, NAIOMT, PNF, Mulligan and McKenzie techniques.
PHYSICAL THERAPY VISIT DETAILS
All sessions will be an individualized and customized 1-on-1 experience with a Doctor of Physical Therapy.
FIRST VISIT: YOUR EVALUATION
This visit usually focuses on movement assessment, as we need to identify pain producers, understand areas of limitation and dysfunction, so we know where to focus treatment. Specific treatment will follow the assessment. At the end of this session, you will be given a tailored home program, so you can continue to work towards long-lasting improvement and progress towards achieving your long-term goals.
At the end of your first session, the therapist will recommend a follow-up visit package size that would be best to relieve your pain and make long lasting changes.
SUBSEQUENT VISITS
Usually, these begin with a shorter assessment to determine if we are heading in the correct direction, if progression is needed or if we need to change course. This session will be mostly treatment techniques and testing to accurately identify the correct progression level for your home program.
INTAKE FORMS
After your first visit is scheduled, you will be sent an intake form to fill out, print and bring to your first visit. This will include a thorough medical history form and standard policies and consent to treat information.
DRESS
Please dress in active clothing that allows your body to move. This allows us to be more accurate with your assessment and provide better treatment.
PAYMENT OPTIONS
PhysioMender is an Out-of-Network, fee for service provider. Payment is due at the time of service. Cash, check, credit card, HSA, FSA are accepted forms of payment. We do not directly accept insurance. You can check with you insurance company to see if you have Out-of-Network benefits, and if so, you may request a receipt/ Super Bill (after all visits for the given plan of care are completed) to submit for reimbursement to your insurance company. You CANNOT submit Out-of-Network claims to Medicare.
